Job Description

• Experience: Minimum of two (2) years of experience in a home health setting within the last ten (10) years.
• License: Current RN licensure in New York is required.
• Certifications: Must obtain BLS certification prior to the start date.

This role requires experience in conducting comprehensive assessments, providing primary care for homebound individuals, and familiarity with criteria like OASIS. The ideal candidate will have:

• Hands-on experience conducting assessments.
• A background in caring for geriatric populations and managing chronic diseases.
• Strong communication skills to discuss healthcare options and collaborate with interdisciplinary teams.

Key Responsibilities:

• Perform initial in-home evaluations of patients, assessing health status, strengths, needs, and preferences.
• Develop individualized long-term care service plans based on evaluations.
• Review medical documentation and health referral forms, documenting any concerns during the evaluation process.
• Ensure continuity of care and identify opportunities for health promotion and illness prevention.
• Collect quality review data to support outcome measurements.
• Enhance collaborative relationships with care team members and maintain a working knowledge of community resources.
